Jasper County Health Department-Behavioral Health is moving.
We are very excited to announce that the Behavioral Health department (Newton office) will be moving from 106 Edwards Street to 910 S Van Buren Street in Newton. Our office will be closed from March 27 through March 31st. We will reopen in our new location at 910 S Van Buren St, Newton, IL 62448 on Monday April 3rd. Please be patient as we transition during this time. We are very excited to have additional space in our new location. Our phone number and mailing/business address will remain the same.   • To register call 618-783-4436.
  • Anyone 5 years or older can schedule an appointment for a Covid-19 Vaccination at the Jasper County Health Department office at 106 East Edwards street in Newton, IL.
  • Download and complete (please use legal name) the Vaccine Consent Form
  • Read the appropriate FACT SHEET (available below)
  • A parent or legal guardian will need to sign the Vaccine Consent Form for anyone 5 years or older.
  • Bring the completed Vaccine Consent Form and your insurance card to your appointment.
