Jasper County Health Department Covid Vaccination Clinics (any individual 18 years of age or older)
April 1st from 9 am to 11:45 am Newton Community High School. This clinic will be for 1st shots or 2nd shots. If you register for a FIRST shot, you will receive a Johnson & Johnson shot. If you register for a SECOND shot, you will receive a Moderna. (Please be sure that your first shot was a Moderna shot. Required to Receive Vaccine— • Register for FIRST dose: 1. Read the Johnson & Johnson Fact Sheet - Click here 2. Download and complete (please use legal name) the Vaccine Consent Form 3. Download and print a copy of the ticket. 4.
